Treatment of Personal Information of the Personnel

The Company will treat personal information of directors and employees (staff, part-time employees, associate employees, temporary employees, employees transferred to the Company temporarily, dispatched employees), persons who intend or are intended to be directors or employees of the Company (hereinafter referred to as the “Employment Applicants, etc.”) and retired employees (hereinafter referred to as the “Personal Information of the Personnel”) out of personal information that the Company treats as follows.

1. Usage Purpose of Personal Information of the Personnel

The Company will use Personal Information of the Personnel of the under-mentioned content for the following purposes respectively.

1) Usage purpose of directors and employees

Personnel information Performance rating, educational background, qualification/license, position, employment history, discipline/commendation, etc. For grasp of capable persons, decision of assignment destination/responsible duty, temporary transfer/permanent transfer, training/ability development, promotion/demotion, and other employment management.
Information on family and relatives Family structure, living together or not, existence of dependent family, health condition, etc. For decision of salary, etc., withholding procedures, procedures on social insurance, child care/family care leave, etc., welfare, and other employment management.
Information on body and health Health condition, medical history, mental and physical disorder, results of medical check, etc. For health care, securing of proper working environment, leave of absence, etc., decision of assignment destination, working hours management, and other employment management.

2) Usage Purpose of Personal Information of Employment Applicants, etc.

4) Regarding Joint Use

a. Joint use for business contact

Extent of joint user SoftBank Group Corp., SoftBank Corp., and other SoftBank group companies (hereinafter referred to as the “Joint User” ) and subcontracting companies who have concluded a business subcontracting agreement with the Joint Users (including contractors).

b. Joint use for business operation

2. Regarding Disclosure, Revision, etc. and Suspension, etc. of Use of Personal Information of Personnel

With regard to presentation of request for and complaint with regard to Notification of Usage Purpose; Disclosure, Revision, Addition, or Deletion (hereinafter referred to the Revision, etc.) and Suspension of Use; Elimination or Suspension of Provision to a Third Party (hereinafter referred to as the Suspension of Use, etc.) of personal information of the personnel; contact information; and required procedures are as follows.

2) Disclosure Procedures of Personal Information of Personnel
  1. a. The Company will send the Application for Disclosure of Personal Information prepared by the Company and a guidance for procedures indicating the address to which the said Application shall be sent, amount of charge, and payment terms to a person who requested disclosure to the said office.
  2. b. A requesting person who has received the said Application is requested to pay the charge by means prescribed by the Company as described in the guidance for procedures and send the said Application after filling in necessary matters and identity verification prescribed by the Company of the requesting person to the designated address. In the event of application by an agent, the identity verification of the agent shall be sent together.
  3. c. In the event that the office to which the Application prescribed by the Company is to be sent has received the said Application filled in with the necessary matters and the identity verification and confirmed payment of the charge, the Company will start the procedures. However, in the event the Application falls under any of the following reasons (hereinafter referred to as the “Nondisclosure Reasons” ), the Company may not comply with the whole or part of the Application.

3) Procedures for Revision and Suspension of Use of Personal Information of Personnel
  1. a. With regard to request for revision to the office concerned, in the event that the matter good enough to specify the information as the object is presented by the requesting person and the Company considers it is necessary to accomplish the usage purpose, the Company will make revision based on the results thereof.
  2. b. With regard to request for suspension of use to the office concerned, in the event that the matter good enough to specify the information as the object is presented by the requesting person, and when the request is found to have a reason, the Company will carry out suspension of use. However, the request falls under the Nondisclosure Reasons, the Company may not comply with the whole or part of the request:
  3. – Case where it is apprehended that life, body, property, and other rights and profit of the person concerned or a third party might be damaged;
  4. – Case where it is apprehended that the proper accomplishment of business of the Company might be troubled substantially; or
  5. – Case where it violates other laws and regulations.

4) Charge pertaining to Disclosure of Personal Information of Personnel and Payment Terms

The charge pertaining to disclosure of personal information shall be 1,080 yen (inclusive of consumption tax) per application. However, it may not be collected in the event that the Company specially admits it. Payment of the charge shall be made by means prescribed by the Company.

Supplementary Provision

– This Regarding Treatment of Personal Information of Personnel is in force as of April 1, 2014.
